甲状腺舌骨囊肿通常位于颈部中线、舌骨下，呈园形，直径约2－3厘米，表面光滑无压痛。检查时囊肿固定，不能向上及左右推移，但吞咽或伸舌时肿块向上移动为其特征。大而浅表的囊肿透光试验阳性，较小的囊肿可扪到一条索带连接舌骨。在青春期，由于囊内分泌物潴留或并发感染，囊肿可破溃形成瘘管，瘘管可向上延伸，紧贴舌骨前后或穿过舌骨直达盲孔，由瘘口经常排出半透明粘液，经过一段时间后，瘘管可暂时愈合而结痂，不久又因分泌物潴留而破溃，这样时发时愈，在瘘口上方可扪及一条向舌骨方向潜行的索带组织。
